Client Consent, Confidentiality & Service Agreement
Welcome to S.O.C.H. Therapeutic Care!
To provide effective counselling and psychological services, it is important for you (“Client”) to understand the rights and responsibilities shared between Therapist and the Client.
Please read this document carefully before signing.
Confidentiality
S.O.C.H Therapeutic Care shall maintain strict confidentiality of all information shared during assessments, counselling sessions, or communication, except in the following circumstances:
- When disclosure is ordered by a court of law.
- When there is an imminent danger to the health or safety of the Client, another individual, or the public.
- When, in the Therapist’s professional opinion, the information suggests a risk of bodily harm to the Client, another person, or the society at large.
- When information is requested by law enforcement authorities in accordance with the law.
- When disclosure is legally mandated under any applicable statute or regulation.
Administrative Access
S.O.C.H. Therapeutic Care may have administrative staff who access limited information strictly for:
- Scheduling
- Billing
- Operational support
All such individuals are bound by confidentiality obligations.
Clients Below 18 Years (Minors)
If the Client is a minor (below 18 years of age):
- S.O.C.H. may share limited information with the parent(s)/guardian(s) only if the Therapist believes it is in the minor’s best interest.
- Before sharing such information, the Therapist may discuss the matter with the minor and consider their comfort, safety, and objections.
Emergency & Crisis Disclaimer
S.O.C.H. Therapeutic Care is not:
- A suicide-prevention helpline
- An emergency service provider
- A platform for crisis intervention
- A provider of psychiatric medication
If the Client is experiencing severe distress, suicidal thoughts, or any emergency, they must immediately contact the relevant emergency helpline.

Refund Policy
S.O.C.H. Therapeutic Care is a service-based mental health practice.
As time and professional resources are allocated specifically for each session:
Once a service has been provided, no refunds will be issued.
We encourage clients to review service details before booking.

- I am above 18 years of age on the date of signing.
- I am mentally sound and signing voluntarily, without fear or pressure.
- I have carefully read and understood this document.
- I have read and understood the Terms and Conditions of SOCH Therapeutic Care available on its official platforms.
- I agree to comply with all terms, policies, and procedures of SOCH Therapeutic Care.
